Ready to schedule an intake? We look forward to meeting you or your referral! While we understand the importance of a swift and efficient referral process, we do routinely maintain a wait list for our residential program. Please complete the following documents and submit them to our admissions team at [email protected]. Upon receipt of all required documents, you or your referral will be placed on our wait list.

Pre-Intake form
Required for all women seeking treatment at Meta House. This document helps us gather essential information so we can begin to tailor our approach to her specific needs.
COVid + client expectations questionnaire
Ensuring the safety and wellbeing of our clients and staff is our top priority. This questionnaire helps us gain insight into potential COVID exposure and outlines the guidelines clients can expect to see while in our program.
OBgyn / prenatal referral
If a client is pregnant at the time of intake, we strongly that all prenatal care gets transferred to a nearby provider. Referrals should be in progress or complete by the date of intake to ensure women have access to critical services.
The safety and wellbeing of children accompanying their mother in treatment is of utmost importance. Completion of this form is required for all women who are bringing a child or children with them into residential treatment.
Child Checklist
Required for women who are bringing a child/children with her to residential treatment. Meta House can provide infant supplies that are acquired chiefly through donation, and certain supplies are limited. To ensure we’re meeting essential needs of the child, please complete the child checklist.
